Joining up Health and Social Care in Forth Valley
Integration of Health and Social Care is the Scottish Government's programme of reform to improve services for people who use Adult Health and Social Care Services.
Stirling Council, Clackmannanshire Council and NHS Forth Valley Health Board are working together on a dramatically different approach to providing the ambitious programme of reform.
Health and Social Care Integration is all about putting people first and ensuring that services are tailored to meet the individual needs of Service Users and their families.
You won't need to do anything differently - services will be more joined-up behind the scenes. Health and Social Care staff will work more closely to support individuals and deliver joined-up care in local communities across Forth Valley.
