Before you begin
Make sure you're ready to provide details of:
- the exact location of the incident, with reference to an adjacent building or landmark
- a full description of the defect, with measurements
- the time, date and weather conditions
- the direction of travel
- why you believe the highway authority is responsible for the accident
- details of the injury that you sustained
- your date of birth
- details of any treating doctor or NHS hospital, if that applies
- details of your earnings, if your claim involves a loss of earnings
- information on the damaged property, including an indication of the original purchase price and date of purchase
- written estimates for the repair or replacement of the damaged property
You should also be ready to upload:
- photographs of the alleged road or footway defect
- receipts from the purchase of the property that's been damaged, if you have these
If you need to, gather all of this information before you begin. You will not be able to save your progress once you start filling in the form.
